چکیده

Renovascular occlusive lesions account for the most common form of surgically correctable hypertension. Technical advances in vascular reconstructive surgery have increased the likelihood of renal parenchymal preservation and amelioration of hypertension in patients having renal artery stenoses. Proper selection of patients alleged to have renovascular hypertension for operative treatment is the most important determinant of a successful result. Measurements of systemic and renal vein renin activity have been used to define the functional significance of suspected renal artery occlusive lesions. Ischemic kidney renin hypersecretion associated with contralateral renin suppression has recently evolved as a potentially reliable index of surgical curability [24]. The present investigation was undertaken to determine the discriminatory value of two expressions of excessive plasma renin activity. The reliability of systemic:renin indices (RSRI) and renal vein renin ratios (RVRR) in predicting responses to operation for remedial renovascular hypertension was assessed.

Renovascular hypertension with low-to-normal plasma renin: clinical and angiographic features.

1. Low-to-normal renin renovascular hypertension (RVH) and the accuracy of renal vein renin ratios were investigated in 129 consecutive patients referred for suspected RVH. Forty-nine had essential hypertension, 24 unilateral renoparenchymal hypertension and 56 renal artery obstruction. Of the latter, 86% were diagnosed retrospectively as RVH, based on fall in blood pressure with correction of ...

Prospective analysis of strategies for diagnosing renovascular hypertension.

Renovascular hypertension is a potentially curable form of high blood pressure. However, it is unclear how best to select patients who are likely to have renovascular hypertension, what diagnostic strategy to use in these selected patients, and how to predict the hemodynamic significance of a renal artery stenosis. We determined the prevalence of renovascular hypertension in adults who exhibite...

Relation between renin release and blood pressure response to nonsteroidal anti-inflammatory drugs in hypertension.

This issue of Hypertension features an article by Imanishi and colleagues entitled "Aspirin Lowers Blood Pressure in Patients With Renovascular Hypertension." In their study, Imanishi and coworkers compared the effects of intravenously administered aspirin on arterial blood pressure, plasma renin activity in the renal veins and aorta, and plasma levels of prostaglandin E2 in the renal veins and...

Renovascular Hypertension: Pathophysiology, Diagnosis, and Treatment

Renovascular hypertension can result from renal artery lesions involving the main renal artery, or its branches. It is generally felt that the elevation of blood pressure results from excessive systemic vasoconstriction secondary to enhanced renin secretion by one or part of one kidney. Renin secretion is enhanced because of constriction of the renal artery and resultant intrarenal ischemia. Cl...

Control of enzymatically inactive renin in man under various pathological conditions: implications for the interpretation of renin measurements in peripheral and renal venous plasma.

1. Human plasma contains two types of renin: one is active in its native form (active renin), the other has renin-like activity after exposure to low pH (inactive renin). Reactions of acid-activated plasma renin and kidney renin with either homo logous or heterologous substrate showed identical Km values. 2. Peripheral venous values for active and inactive renin in essential hypertension (n = ...
